Community Health Needs Assessment
Your Health is Our Mission
We’re passionate about improving the health and wellness of our communities. That means offering world-class care and the resources you need to get and stay healthy.
Guide Our Way
Community Health Needs Assessments (CHNAs) are conducted to help us better understand the unique health needs of our communities. By working with a diverse group of community leaders and the Georgia Health Policy Center, we can collect comprehensive data for our CHNA.
CHNAs pinpoint the most vital health needs of our community, informing our next steps and helping us create a blueprint for improving care and promoting healthy lifestyles.
We make these documents available to comply with the Internal Revenue Service and the Affordable Care Act (ACA) tax law requirements section 501(r), which requires hospital facilities owned and operated by an organization described in Code section 501(c)(3) to conduct a CHNA once every three years.
