Request Your Medical Records
As a patient, it’s your right to get copies of most medical records. If you’ve received care at a Wellstar hospital or see a Wellstar doctor for ongoing care, you may access your medical records, by request, at any time.
We’ve done our part to make requesting your medical information easy. With your request—through MyChart, email, fax or mail—we'll provide you with doctor's notes, medical test results, lab reports and billing information.
Accessing Your Personal Health Information
If you wish to obtain medical records for your personal use, we are happy to assist you in this process. Please see below for the costs of each type of record delivery:
- Electronic records delivered in electronic format (secured email) — $6.50
- Electronic records delivered via active MyChart account — $0.00
- Paper records sent via mail — $6.50 plus postage and taxes
*Sales tax and postage as applicable will be charged for medical records per Georgia Statute 45 CFR 164.524(c)(4) and O.C.G.A. 31-333.
Authorization for the Release of Protected Health Information
All requests for medical records must be fully completed and dated on or after the date of discharge to be processed. After the properly executed authorization has been received, our team will review for all required elements and processes in accordance with federal and state laws governing you privacy and medical records. Please allow 3 to 5 days for this process. For your convenience we offer delivery electronically, or records can be mailed to the address listed on the authorization. Please see e-request form under ROI documents.
Your Rights and Access to Personal Health Information
Wellstar Health System recognizes the patient's right to access and obtain copies of their protected health information (PHI) in accordance to HIPAA laws. (Code of Federal Regulation 164) as well as Georgia Statutes. Should you require any behavioral health records, O.C.G.A. 31-33-2 requires physician approval prior to us releasing any medical records to you. Please allow an additional 3 to 4 business days for these requests to be processed.
Records for your physician
If your physician is on staff at a Wellstar Hospital they may access your medical records from their office without you facilitating this request. If the office has requested that you personally request the medical records from a Wellstar Health System Hospital then we can fax the records to your physician's office at no charge to you, once you complete the authorization for the Release of Protected Health Information Form in detail ensuring the following items are included:
- Physician Name
- Complete Address
- Phone Number
- Fax Number
A form must be completed for each physician who may need your records mailed or faxed. Please note that your records will be mailed unless you specify an appointment time and date. If specified the records will be faxed.
Insurance Requests/Attorney Requests/Disability Requests
Requests should be sent from insurance companies, attorney or Departments of Disability Determination Services and, mailed to the address on our authorization. All charges for medical records will be billed to the requestor.
Radiology/Imaging Films
Please contact the department directly that performs that service to facilitate your request.
MyChart Patient Portal
If you already have an active MyChart account with Wellstar, you can request a copy at no cost of your records.
- Sign on to MyChart
- Select Menu in the upper-left corner, then choose Document Center (in "My Record" section)
- Select Visit Records and then select one of the options below:
- Single Visit - Select a single visit
- Date Range – Select a date range
- All Visits
Options available to view medical records:
- View - See medical records on-screen in MyChart
- Download - Download a zip file and save on your device/computer – Password protection is available
- Send - Send to a provider of your choice
If you do not have an active MyChart account with Wellstar:
- On the MyChart main web page, click on Sign Up (I don't have a code).
- Complete the requested information and click Submit. You will receive an email back with your activation code on how to activate your MyChart account.
- Once you can log into MyChart, follow the previous steps on submitting a request for your medical records.

Records Requests via MyChart are limited to the following dates
Records dated April 1, 2014 and forward are available via MyChart for the following hospitals: Cobb, Douglas, Kennestone, Paulding, and Windy Hill.
Records dated February 25, 2018 and forward are available via MyChart for the following hospitals: Atlanta Medical Center Main/South, North Fulton, Spalding, Sylvan Grove and West Georgia.
Records for your medical office will vary.
Online Patient Request Tool
Wellstar Health System now offers an online records request tool for complete copies of your medical records. You will need to upload a copy of your photo ID, which can be taken via webcam or a smartphone device. This online tool will let you request all of your records and radiology images. While there is no fee to use this electronic tool, to receive the records electronically, there is a $6.50 fee. Chrome, Safari and Firefox are the recommended browsers for this application.

Please note that physical mail is no longer being received at Wellstar Atlanta Medical Center or Wellstar Atlanta Medical Center South. To request records from these locations, please use the e-request and/or mail at other Wellstar locations.
