Do not use this App or its digital services for medical emergencies. Call 911 immediately if you experience a medical emergency, or have someone take you to an emergency department if you or another person needs immediate medical assistance. You agree that WHS is not responsible or liable in any way for any delays or barriers to care access and/or treatment caused by use of this App, and that WHS not be held responsible for any damages arising from use of this App. If you are dissatisfied with this App, you sole remedy is to stop using it.
Depending on your preference, this App will sort provider and facility distances from your immediate perceived or specified location. Use of this feature is at your own risk and our location data may be inaccurate. Do not rely on App location services during emergencies.

All health-related information provided via the App is intended to educate and inform users about the scope of WHS' services; facilitate non-emergency/non-critical connections to providers and medical services; help users learn ways to get well, live well and stay well; and give users improved ways to connect with WHS.
This App is not a substitute for medical advice and does not imply the delivery of medical advice or any assistance with any type of diagnosis. Users should not use the App as a substitute for a consultation with a healthcare provider.
Although WHS has attempted to produce an error-free App, WHS cannot guarantee accuracy or functionality of the App. If you discover an error or experience a bug, please contact our development team. WHS frequently revises its policies and in-App features and information without versioning the App. WHS may update or change the App, its features, services and analytics criteria without notice; these changes apply to all information collected prior to the changes and at the time of the changes, as well as any information generated, created or received after the changes are established. Please review this EULA often to learn about changes that may affect you.
Effective and ethical data collection and usage are increasingly important in healthcare. Earning and keeping your trust is important to us, and we hope you'll count on WHS to partner with you to get, stay and live well. To assist in this journey, we track many different types of data using the App, like how frequently it's downloaded and used; what features users of our community prefer; if the App positively contributes to general patient goals; locations in the community that would benefit from better access to specific health services; and many other datasets that are used to make decisions on what features and services WHS offers in the future. Although WHS has used reasonable precautions to safeguard the confidentiality of information received and sent through this App, WHS cannot guarantee the confidentiality of such information.
The App does not directly provide WHS with any protected health information (PHI) about a user. If you decide to access MyChart, ER Express check-ins, ZocDoc appointment scheduling or other third-party tools through the App, please note that you will leave the App and be subject to terms, conditions and policies associated with those applications, and those apps may provide WHS with PHI.
