Community Health Needs Assessment (CHNA)
and Implementation Strategy

At WellStar Health System, we are committed to improving the health and wellness of our community through providing world-class care, as well as offering resources that promote healthy lifestyles.


As part of that commitment, Community Health Needs Assessments (CHNA) are conducted to better understand, identify and prioritize the health needs of the communities served by WellStar Health System’s (“WellStar”) hospitals. By collaborating with a diverse coalition of community stakeholders and partnering with the Georgia Health Policy Center, we drive the CHNA with systematic, comprehensive data collection and analysis.


The CHNA is accompanied by an Implementation Strategy, which is an actionable blueprint that addresses the prioritized health needs of the community to improve access to care and promote healthy lifestyles.

Any feedback on the CHNA can be addressed to chna@wellstar.org. These documents are made publicly available in compliance with the Internal Revenue Service and the Affordable Care Act (ACA) tax law requirements section 501(r). The law requires hospital facilities owned and operated by an organization described in Code section 501(c)(3) to conduct a CHNA once every three years.
