Prescription Refills

Please request medication refills through MyChart or through your pharmacy. Medications are refilled during normal office hours only. On-call physicians do not refill medications after hours or on weekends. We do not refill medications for patients we have not seen in the office. For patients experiencing chronic pain, we coordinate care with pain management. We do not prescribe routine, ongoing, and continuous narcotic pain medications.
In the event your insurance requires a referral to a specialist, please contact the referring physician’s office or the primary care physician listed on your insurance card and provide the date of your appointment and the physician you will be seeing at our office. We must receive this referral prior to your visit to insure that your insurance processes and pays your benefit accurately. Please have the referral faxed to our office at 770-421-6112.

General Directions

From I-75:

Exit Canton Road toward Marietta (#267B). Travel southbound for approximately 1 mile. Turn left on Vann Street just before Wellstar Kennestone Urgent Care. Go one block and turn right onto Canton Road. Building 780 is on the left approximately 1 block from Vann Street. Enter through the building front entrance and proceed to the 4th floor. We are suite 400. Parking is free. Please allow ample time for travel and parking.

Appointments & Cancellations

Please call 770-422-3602 and select Option 1 to schedule or cancel appointments. You may also use MyChart to communicate with our office. You must notify us 24 hours in advance if you need to reschedule or cancel an appointment. Late cancellations/reschedules are considered a “no show” for scheduled appointments. If your insurance provider requires a referral, please ensure that a referral has been provided to our office prior to the time of the appointment. If you have completed MRI, X-ray, or CT scan(s) prior to your appointment, please bring a copy of these diagnostic tests with you to the appointment.

Please bring all medications you are taking with you to the appointment. If you prefer, you may bring a detailed list of medications. This list must be complete and include medication name, dosage, and frequency (how you are prescribed to take it and how you are taking it).

Insurance and Payment

All co-payments, deductibles, and self-pay amounts are due at the time of service. We accept cash, checks, and credit cards. Please remember to bring you insurance card and photo ID with you to each visit.

Wellstar Medical Group participates in most major insurance plans. We will file your insurance for you if we are a participating provider of your plan.

About Us

Wellstar Neurology & Headache Center is conveniently located near Kennestone Hospital. Our physicians treat disorders of the nervous system, which includes diseases of the brain, spinal cord, nerves, and muscles. Some common problems neurologists diagnose and treat are strokes, epilepsy (seizures), headaches, dementias (Alzheimer’s), multiple sclerosis, Parkinson’s disease, and various chronic pain conditions. We are a headache center and treat various headache conditions, including migraine.


Board Certified in Neurology, Clinical Neurophysiology, and Electrodiagnostic Medicine.

Dr. Joseph Hormes received his medical degree from St John's Medical School, Bangalore, India. He completed neurology residency at University of Colorado Health Sciences Center, Denver, and his Fellowship in Neurophysiology at Mayo Clinic, Rochester.


Board Certified in Neurology and Clinical Neurophysiology with additional subspecialty certification in Neuroimaging.

Nithi S. Anand, MD, MPH completed his 6-year medical school training at the University of Madras, India. He also completed three years of internal medicine training in India. At the University of Massachusetts, Worchester, he completed a 4-year neurology residency, a fellowship in clinical neurophysiology, and served as Chief Resident during his final year of training. He also completed a Master’s in Public Health degree at the University of Massachusetts, Worchester.


Board Certified in Neurology and Clinical Neurophysiology.

Dr. Holmes completed medical school at Duke University. He completed his residency training at Harvard University. He also completed a post-residency fellowship in epilepsy at Emory University.

Dr. Reyzelman attended medical school at the State University of New York (SUNY) at Buffalo. He completed his residency at Emory University, where he served as Chief Resident in his final year of training. He Also completed the Clinical Neurophysiology and Neuromuscular Disease Fellowship program at Emory.
