Jesse Smith, NP-C, is a board-certified family nurse practitioner who provides primary care for patients age 10 and older in Alpharetta, Georgia. He has a special focus on chronic disease prevention and management and is passionate about helping patients maintain long-term health and wellness.

Jesse earned his family nurse practitioner degree from South University and is certified by the American Academy of Nurse Practitioners (AANP). He believes primary care is the foundation of whole-person lifelong health, preventing illness, promoting wellness, and supporting patients through every stage of life.

In his spare time, Jesse enjoys spending time with his family, exercising, and hiking and participating in other outdoor activities. He welcomes new patients and accepts most insurance plans.

Jignesh R. Dholaria, MD, is a board-certified family medicine physician who provides primary care for patients age 5 and older in Alpharetta, Georgia. He is dedicated to caring for the whole family and believes in motivating patients to take an active role in managing their own health and making lifestyle changes when needed to support long-term wellness.

Dr. Dholaria earned his medical degree from Spartan Health Sciences University School of Medicine and completed his family medicine residency at Memorial Health University Medical Center in Savannah, Georgia. He also completed a fellowship in geriatrics at the Medical Center of Georgia in Macon. Dr. Dholaria is certified by the American Board of Family Medicine and is an active member of the organization.

In his spare time, Dr. Dholaria enjoys traveling, spending time with his family, and playing tennis and pickleball. He welcomes new patients and accepts most insurance plans.
