Wellstar Mobile App: Account and Data Deletion
Requesting Deletion of Your Wellstar Account
Users who have created a Wellstar Account (usually through our Wellstar Mobile App) can delete this account and all its associated data by following the instructions below.
Please note the following:
- Placing a Wellstar Account deletion request is irreversible. It will result in the termination and complete deletion of your Wellstar Account and all associated data (e.g., personal information, unlocked achievements, Wellstar-stored Real Age Test results, etc.)
- After Wellstar Account deletion, you may create a new Wellstar Account at any time through the Wellstar App.
- Wellstar Account deletions are typically processed within three business days.
- The team does not retain records of or provide support for prior account deletions.
- This is NOT a deletion request of your MyChart account or patient record. To deactivate your MyChart account, visit the Security Settings page at mychart.wellstar.org.
- This is NOT a deletion request of your Sharecare account or its data. To delete your Sharecare account and data, visit the Sharecare Privacy Web form.
- This is NOT a deletion request for any CRM user record that is maintained in another Wellstar system.
To proceed with your Wellstar Account deletion request:
- Click this link to generate an email to our account deletion team.
- Your email MUST contain the following information to identify an existing Wellstar Account:
- Email address associated with your Wellstar Account
- First name
- Middle name
- Last name
- Phone number associated with your Wellstar Account
- Zip code
- Date of birth
- Gender
