Carolyn Smallwood, DO, is a board-certified family medicine physician who provides comprehensive, whole-person care for patients age 5 and older. She has a strong commitment to preventive health and values honest, respectful communication as the foundation of a successful patient-provider relationship.
Dr. Smallwood earned her medical degree from the Philadelphia College of Osteopathic Medicine after completing her bachelor’s degree in cross-cultural studies from Toccoa Falls College. She also holds a Master of Public Health with a concentration in environmental health science from A.T. Still University. She completed her family medicine residency at St. Mary-Corwin Medical Center in Pueblo, Colorado, where she served as Chief Resident. She is a fellow of the American Academy of Family Physicians and a member of the American Board of Family Medicine.
Dr. Smallwood provides a full spectrum of primary care services, with a special focus on preventive care. She is dedicated to staying current with evidence-based research and techniques to ensure high-quality, patient-centered care.
Outside of work, Dr. Smallwood enjoys spending time with her husband and three sons. She loves traveling, gardening, cooking, and reading. She welcomes new patients and accepts most insurance plans.

Dr. Smallwood has great listening skills. She is thorough. She tells me what I need to hear in a clear and respectful way. I never feel rushed nor insulted. I highly recommend Dr. Smallwood.

This is my first time visiting a PCP in two decades and I was nervous. She showed the most care and concern of any provider I've ever been a patient of. It was refreshing that she didn't just focus on my physical health but also my metal and emotional health as well. I feel confident being in her care.

My experience was wonderful as it always is. The front staff is always kind and courteous and greets me with a smile. My wait time was very little, which I appreciated. And of course, Dr. Smallwood is always five stars. She is fabulous. And personally, I wish every person could have a doctor as awesome as she. I appreciate her so much and she listens and she truly cares. And I just really appreciate her more than I can explain. I've told several people since being at my appointment how wonderful of a doctor I have. And so great, great experience once again. Thank you.
