Cardiac Rehabilitation Services

Cardiac Continuum:  SECONDARY PREVENTION

Cardiac Rehabilitation Services’ medically supervised programs focus on helping heart disease patients maintain a healthier heart.   Cardiologists and the cardiac rehabilitation team tailor cardiac rehab programs to meet each patient’s specific needs.

Services include:

Inpatient Cardiac Education
Cardiac Education is designed to meet cardiac patients’ educational and emotional needs while at the hospital.  This Phase 1 rehab provides patients and families with information about cardiac diagnoses, tests and procedures.  Rehab professionals teach patients how to live a heart-healthy life, not only for the duration of the rehab, but also for the rest of their lives.  Phase 1 prepares patients for a smooth transition into outpatient care and recovery and provides the necessary resources to continue education and control risk factors.

Outpatient Exercise Program
The Outpatient Exercise Program is a medically supervised program to help strengthen the heart.  Classes are designed to provide support, information and ongoing education for a heart-healthy lifestyle including successful risk factor modification.  Cost and length of the program are based on physician referral and the needs of the patient.  Insurance may cover some or all of the cost.

Heart Smart School
Heart Smart School is a free education class for patients, family and friends taught by a cardiac nurse, an exercise specialist and a dietician.  Topics include how the heart works, risk factor modification for a heart healthy lifestyle, cardiac nutrition, exercise, and the emotional aspects of heart disease.  The class is ideal for congestive heart failure patients, people with a high risk for heart disease or those diagnosed with heart disease that may or may not have had cardiac intervention/surgery.

Pacemaker/Defibrillator Support Group
The Pacemaker/Defibrillator Support Group is a free, quarterly event for patients and their families who either have a device or may need one in the future.  The event is facilitated in a group setting and encourages participants to share personal experiences.  The latest advances in PM/ICD technology also are reviewed.

Congestive Heart Failure Support Group
This support group for congestive heart failure patients meets quarterly for support and ongoing education related to their disease.

Heart of the Matter
The Heart of the Matter class addresses the psychological and emotional aspects of living with heart disease for the patient and their family.  The bi-monthy class is facilitated by a certified clinical psychologist specializing in cardiac-related issues.

Mended Hearts
Mended Hearts
is a cardiac support group for patients and families dealing with heart disease and the issues surrounding the disease.
