Hospice at Home
WellStar offers comprehensive onsite hospice services for patients living in their own homes, at a relative’s home or at a skilled nursing or assisted living facility. Team members provide medical services and comfort care and support the family caregivers. We serve six area counties – Bartow, Carroll, Cherokee, Cobb, Douglas and Paulding – with 24-hour on call service by a registered nurse who will coach over the phone, comfort, reassure and provide onsite care as needed.
For many patients, home is an ideal setting for transitioning to hospice care. The services and philosophy are the same ones that guide WellStar’s two inpatient facilities. Service is delivered according to the patient’s condition and needs as determined by an assessment by an RN.
Services include all aspects of pain and symptom management, nursing care, comfort care, medication monitoring and dosage and assessment and selection of needed equipment. We also offer counseling and emotional support and assistance with meals, bathing and other activities of daily living. Our nurses and clinical care partners work closely with family caregivers to provide coaching, instruction and support.
Family members are encouraged to visit often in the home, the patient’s current residence or the inpatient facility. They often become an important part of the care team. Their love and support are among the most important factors in a patient’s level of comfort and ease. By getting to know the family, our staff members get a better understanding of the patient’s preferences and interests. That partnership is essential to the patient’s wellbeing and the family’s peace of mind.
