About 25.8 million people in the United States have diabetes - 8.3% of the population. An estimated 79 million people have pre-diabetes.

Chronic Condition Management

WellStar's Chronic Condition Management program is a free telephonic self-management program for people who meet specific criteria. Between doctor visits, a registered nurse and a licensed social worker act as health coaches to educate, encourage, and support you in managing your diabetes. These calls preserve and support the relationship with your doctor and help you become an active participant in your healthcare.


Social Services

WellStar's Licensed Master of Social Work in Diabetes Services can help you work through the financial, emotional, and interpersonal barriers to good management of your diabetes.


Support Groups

If you or family members are affected by diabetes, you are invited to WellStar's free diabetes support groups, which meet monthly (except December) at WellStar Cobb, Douglas, KennestonePaulding and Towne Lake locations. You will help choose the themes, topics, and speakers for the meetings. This is a chance to share information, learn the latest about diabetes, and review your challenges and successes. Registration is not required.
