General Directions

From I-75: Take Canton Road exit (#267A). Turn right onto Canton Road (Hwy. 5). Take a right at the first light onto Sandy Plains Road. Take a right onto Shallowford Road. Office is located 1.7 miles on the left at the corner of Lassiter and Shallowford roads.
 
From Roswell: Take 92 South. Turn left on Mabry Rd. Go to the end and turn right on Shallowford Rd. Go 1/4 mile and turn right into the parking lot before Lassiter Rd. intersection.

Our phones are open between the hours of 8:00am to 4:45pm. We request 24 hour cancellation notice. As a courtesy, you will receive an automated call to remind you of your appointment 2 days in advance.

Insurance and Payment

We are committed to meeting your health care needs. Our goal is to keep your insurance or other financial arrangements as simple as possible.

WellStar Medical Group participates in most major insurance plans. For a complete list of insurance participants at this practice please call the practice main line. We will file your insurance for you if we are a participating provider of your plan.

All co-payments are due at the time of service. Please remember to bring your insurance card (HMO, CMO, PPO, etc.) with you to each appointment.

Hospitalization

Our doctors have privileges at WellStar Kennestone Hospital. If you are admitted as an in-patient at WellStar Kennestone Hospital, you will be seen by a Hospitalist physician who will communicate with your Primary Care Physician.
 
A Hospitalist is a physician who specializes in caring for patients in the hospital 24 hours a day. They are Board Certified Internal Medicine Physicians who will be responsible for your care from admission through discharge.

The Cobb Diabetes Support Group meets the third Wednesday of the month at our office from 6:00 pm to 7:30 pm. Please join this free opportunity to learn more about diabetes.
