To schedule an appointment please call our appointment line: (678) 354-0455 and press 2.
Our phones are open from 7:00 AM - 4:00 PM Monday - Thursday with Lunch from 11:15 AM - 12:15 PM and Fridays from 7:00 AM – 12:00 PM.
After hours and on holidays our phones are forwarded to the answering service. Please note we do not refill medications, prescribe antibiotics, or pain medications after hours. Please call our office the next morning for a same day appointment or proceed to Urgent Care.
If you would like to request a non-urgent appointment or cancel an appointment more than 2 business days in advance, we offer online appointment requests by clicking the link on this web page.
We request 24 hour cancellation notice.
We believe having your blood test results at the time of your visit is the best way to answer all your questions and to make sure that you receive the most appropriate treatment.
Please have your lab work done at least two days before your scheduled appointment. You can stop by our office to have your blood drawn. If you call and let us know when you are coming, we can have you on your way in about 10 minutes.
WellStar Cornerstone will not collect a co pay at the time you have your blood drawn, but some insurance plans may charge a "threshold co-payment." If your insurance company charges this co-payment, you will be billed for it. (Refer to your insurance companys benefits manual for details.)
You may also have your blood drawn at any LabCorp facility (www.LabCorp.com). You do not need an appointment but you will need an order form. Please call our office, and we will get your orders to you or to the lab.
Please check with your insurance company to see if they want you to use a Quest (www.questdiagnostics.com) or a LabCorp site.
Please don’t call for results before your office visit. Your results will be faxed to our office, and we will discuss them during your visit.
Reminders:
Fasting means no food or drinks other than water for 8 hours before your blood test. IT IS ALWAYS OKAY TO DRINK WATER, this will not affect any of your tests. You only need to fast for cholesterol and glucose (sugar) screenings. Hemoglobin A1C is NOT a fasting test.

Hospitalization
Our doctors have privileges at Cobb and Kennestone Hospitals. If you are admitted as an in-patient at Cobb or Kennestone, you will be seen by a Hospitalist physician who will communicate with your Primary Care Physician.
A Hospitalist is a physician who specializes in caring for patients in the hospital 24 hours a day. They are Board Certified Internal Medicine Physicians who will be responsible for your care from admission through discharge.

Just click on the form, fill it out and click on submit. We’ll have the information entered into our system before you arrive for your appointment!
Please note we will need your picture ID (drivers license) and Insurance card at each visit, please have these ready for us when you arrive.
