Prescription Refills
Please have your pharmacist fax your refill request to (678)354-3469 or (678)354-6887.
 
FOR CURRENT PATIENTS ONLY: If you would like to request a prescription refill online, you may click on the prescription refill link on this web page. In addition to entering the pharmacy number in the appropriate box, please also include the pharmacy fax number in that same box. Please separate the two numbers by a slash. ex. 770-648-2156/770-648-2159
 
Please allow 24 to 48 hours for your refill request to be filled.

If you would like to request a referral on line you may click on the referral link on this web page.
 
Referrals will be complete within 3 days.
 
For all non-urgent new referrals, you may need to make an appointment to see your Primary Care Physician before the referral will be completed.

General Directions

From I-75: Take the Canton Road/Hwy. 5 exit (#267B) toward Marietta. Pass WellStar Kennestone Hospital on the right and continue on Church Street until Ardis Street. Once you cross the 120 loop, turn left on Ardis (second left) and we are located at the corner of Cherokee and Ardis streets. Parking is located off Ardis in the back of the building.

Our phones are open between the hours of 7:30am to 4:00pm. During the hours of 4:00pm to 7:30am our phones are on answering service for emergencies.

Please call 24 hours in advance if you need to cancel or reschedule an appointment. There is a $25.00 No-Show charge.

Hospitalization

Our physicians have privileges at Kennestone Hospital. If you are admitted as an in-patient at Kennestone Hospital, you will be seen by a hospitalist physician who will communicate with your Primary Care Physician.
 
A Hospitalist is a physician who specializes in caring for patients in the hospital 24 hours a day. They are Board Certified Internal Medicine Physicians who will be responsible for your care from admission through discharge.
