Prescription Refills
We strive to provide you with enough refills to last until you are due for your next appointment. Please bring all medications with you to each visit. If you are in need of a refill, you are most likely due for an appointment. Please plan to schedule an appointment prior to running out of your medication. We realize there are exceptions, therefore we ask that you call our office and speak with a member of our clinical staff to request your refill. Prescription refill requests will be addressed on any individual basis. We strive to handle your request as quickly as possible however, please allow at least 48 hours. We will only refill medications if you are a current patient of the practice.
Referrals
FOR CURRENT PATIENTS ONLY - If you would like to request a referral, please call our main line number at (770) 974-4655. Referrals will be completed within 3-5 business days.

General Directions

Located at the intersection of Highway 41 and Cedarcrest Road, approximately ½ mile south of the Bartow County line.

From I-75:
Take exit 278 for Glade Road toward Acworth. Drive for .4 miles and turn left onto Cherokee Street. Drive .4 miles and turn right onto GA92/Lake Acworth Drive. Drive 2.3 miles and turn right onto Hwy 41/N. Cobb Parkway. The Acworth Health Park will be on your right, located at 4550 N. Cobb Parkway.

From Hwy 41, going North:
Cross Cedarcrest road and turn right at the Acworth Health Park Sign.

From Hwy 41, going South:
Turn left at the traffic light onto Cedarcrest Road. Take your first left and proceed towards the Acworth Health Park.

Appointments & Cancellations

To schedule an appointment please call our office at 770-974-4655 and select option 2. Please contact our office at least 24 hours prior to your appointment time to cancel or reschedule so that we may accommodate other patients that need to be seen. We request that you bring all medications with you to each visit.

After hours we share call with other physicians to answer emergency questions.  Prescription refills and appointment requests will not be addressed by the physician on call. Please call during regular office hours for these requests. For emergencies after hours, please contact our office and the recording will provide you with information to reach the physician on call.

Insurance and Payment

We are committed to meeting your health care needs. Our goal is to keep your insurance or other financial arrangements as simple as possible.

WellStar Medical Group participates in most major insurance plans. For a complete list of insurance participants at this practice please call the practice main line. We will file your insurance for you if we are a participating provider of your plan.

All co-payments are due at the time of service. Please remember to bring your insurance card (HMO, CMO, PPO, etc.) with you to each appointment.
