General Directions

From I-20 West: Take the Chapel Hill/Campbellton Street exit (#36). Turn north on Campbellton Street toward Douglasville. At the first light, veer right on Hospital Drive. Go to the second light and turn right on Professional Parkway. Our office is located in the first two-story brick building (A Building) on the right, downstairs in suite 100.

Appointments & Cancellations

Our phones are open between the hours of 8:00am - 12:00pm and 1:00pm - 4:45pm. During the hours of 4:45pm to 8:30am our phones are forwarded to answering service for emergencies. We require 24 hour cancellation notice. Well Visit appointments not kept and not cancelled within 24 hours of the appointment time and date will be assessed a $25 no show fee.

Insurance and Payment

All patients, new or established will be required to present proof of insurance at the time of service. If the office cannot verify insurance coverage, the patient will be expected to pay in full at the time services are rendered. If unable to pay, you will be asked to reschedule your appointment. 

We are committed to meeting your health care needs. Our goal is to keep your insurance or other financial arrangements as simple as possible.

WellStar Medical Group participates in most major insurance plans. For a complete list of insurance participants at this practice please call the practice main line. We will file your insurance for you if we are a participating provider of your plan.

All co-payments are due at the time of service. Please remember to bring your insurance card (HMO, CMO, PPO, etc.) with you to each appointment.

Our physicians have admitting privileges at Wellstar Douglas Hospital.

Creekside Pediatrics, formerly Kaufman Pediatrics, joined the WellStar family in 1999. We look forward to the opportunity to serve you and your child’s healthcare needs with a high standard of medical expertise, patient care and customer service. We have a Board Certified Lactation Consultant on staff to address new mothers’ concerns and needs. We also offer a full line of breastfeeding products for purchase, as well as breast pump rentals.
